Insurance
In order to better serve you, we are now participating in many different insurance plans and will
file for reimbursement. It is our goal to meet all insurance requirements. It is your responsibility to
know the details of your insurance plan, so please inform our staff if your plan requires a special
laboratory.
We participate in

At this time, we are
unable to accept Tenncare or Medicaid. If you have insurance coverage not included on our list, please
address this with our staff when appointment is
scheduled.
Upon each visit, we
ask that the co-pay and/or deductible be paid. We gladly accept personal checks, Mastercard, Visa,
Discover & American Express.
